Clindamycin is no longer recommended for endocarditis prophylaxis for dental procedures because of an increased risk of severe adverse reactions. For patients with a history of severe penicillin hypersensitivity, doxycycline or azithromycin is now recommended.
Institutions with high rates of endocarditis following transcatheter aortic valve implantation (TAVI) or cardiac implantable electronic device (CIED) procedures that use an inguinal approach are advised to consider incorporating antibiotics with enterococcal activity into their endocarditis prophylaxis strategy.
